Just to tell my opinion about this: all the filesystems are done wrong. Databases with primary keys have it right. The fact that a file name is a "smart key" is THE mistake, since smart keys are stupid. So the OS is at fault, but don't blame microsoft alone. CVS(NT) does what it can do to make life bearable.
