[cvsnt] checkout -d foo\bar does not create subdir bar?

I see what you mean.  I think it used to work in CVSNT, not sure when it 
stopped working since I don't use that regularly.  You might want to 
file a bug report on that.  I don't think this would have been 
purposefully restricted when the arbitrary path checkout was disabled, 
but I can see how it could have been disabled along with it (but I could 
be wrong).
